Long Term Conditions

Chronic disease management is done for the following conditions:

Home Prescription Delivery

Home Delivery Medication Service for the housebound and elderly – this service is a free service and is provided for surgery patients who are housebound or are over 65 years of age and have a genuine need for delivery of their medication.

Baby Clinic

There’s now an electronic self-referral system for maternity care. Please follow the link below to complete your referral form at the top of the Trust’s website homepage.

Antenatal Clinic

All Antenatal Services are provided by Bluebell Birth Centre. Please check with the reception for further details.

Child immunisations

Child immunisations – a wide range of advice for parents. Immunisations can be given by the practice nurses, with catch up programmes for children who have missed vaccinations for any reason.

Smoking Cessation Clinic

Family Planning Clinic

The clinicians offer the service of emergency contraception.

Contraception – NHS

Travel Vaccinations and Non-NHS Services

Travel vaccinations are recommended in certain countries. The following vaccinations are provided free of charge on the NHS and are the only* vaccines available at our practice:

  • Typhoid
  • Hepatitis A
  • Diphtheria, Tetanus and Polio

A Travel Risk Assessment Form must be completed in practice at least 6 weeks prior to travel.

*These and other vaccines are also available at Private Travel Clinics – many Boots, Superdrug stores and various independent providers offer vaccinations throughout the Borough.

If you require a copy of your immunisation history – this can be provided free of charge – Please speak to the Admin Team.

The NHS does not pay for all of the services we provide, so patients may be required to pay for some services availed. Please see attached list.

Our fees are in line with BMA-approved national guidelines.

NHS Health Checks

We can carry out NHS health checks to patients who are aged 40 to 74.

Cervical Smears

This appointment will be arranged with the Practice Nurse.

Meon Dental Clinic

Meon Dental Care is under new ownership since November 2021. The new Principal Dentist is Mrs Neha Sahdev. We are now operating safely to pre-pandemic levels and are back to routine services including dental therapy and dental hygiene. www.meondentalcare.co.uk
